On June 20, 2011, CMS posted a change to the mandatory ABN form. This is the required form that must be completed when products or services are expected to be denied by Medicare.
Although originally scheduled to be implemented on September 2011, the official implementation date is November 1, 2011. The updated form will be included in the 2012 Behavioral Health MultiBook and the 2012 ChiroCode DeskBook.
There is a "grace period" until January 1, 2012. After that time, ABNs submitted on the old form will be considered invalid.

How do you know if you have the right form? Look at the lower left hand corner of the form, it should say 3/2011.
